Replaced use of custom 'baseUrl' helper with 'url'
...
Also changed up how base URL setting was being done
by manipulating incoming request URLs instead of
altering then on generation.

- Updated breadcrumb dropdown switchers and back-end sibling code to handle new breadcrumbs.
- Added breadcrumb view composer and EntityContext system to mangage
tracking if in the context of a bookshelf.
